Patient Guide
Admission
1. Planned admission
  • Ask for the Ambulance service if needed
  • Give your admission paper to reception staff & ask for help.
  • Inform about Medi-claim at admission desk and give the necessary documents ( ID/ Policy copy ).
  • Get detail about ward charges and select the desired ward
  • Fill the consent form. ( In case of the Medi-claim write the Name according to the policy )
  • Shifting of the patient to particular ward.
2. Emergency
  • Ask for the Ambulance service if needed.
  • Keep your patient in emergency ward.
  • Treatment & assessment of patient by Doctor. (Don’t be panic and only one or two attendant to be with patient who knows the detail)
  • Get detail about condition of the patient from Doctor.
  • Get detail about ward charges and select the desired ward.
  • Shifting of the patient to particular ward.
  • Fill the consent form & inform about Medi-claim at admission desk and give the necessary documents ( Photo ID/ Policy copy ).
Discharge
  • Discharge procedure will take minimum 2 hr. of time.
  • After discharge intimation wait for paper work to be completed.
  • Settle your bill at Billing desk.
  • Get original discharge summary & investigation reports from the nursing station
  • Get detail about medication & follow-up.
  • Ask for the Ambulance service if needed.
  • You can give your suggestions & feedback by filling FEED-BACK form.
Charges, billing and payment
The patients/relatives are advised to enquire and understand the bed charges as well as treatment charges before deciding for admission
The patients are required to pay advance deposit at the time of admission as specified
Valuables: The patients and the relatives are advised to protect their cash and belongings. The hospital does not accept any responsibility for damage theft or burglary.
